Health Beyond Healthcare

Innovative health care institutions and leaders are heeding the call to move beyond treating illness to helping people lead healthy lives. Changing trends in reimbursement are accelerating this move to incorporate non-medical vital signs—such as employment, education, health literacy, and safe housing—into the assessment and treatment of patients, especially those with complex physical, emotional and social needs. KentuckyOne Health, part of Catholic Health Initiatives, has implemented a novel program called “Health Connections” that is not only significantly improving the health and well-being of participants, but resulting in significant cost savings. Bev Beckman, the project leader whose research shaped the program, will explain the model which has been replicated in Arkansas, Texas, Washington, and even Belize.

Bev Beckman oversees management of Health Connections in Louisville, Ky. and Little Rock, AR.—an innovative community-based care transitions program focused on the most high-risk and high-cost patient population impacted by the social determinants of health. She is a registered nurse with more than 30 years of progressive experience in hospital and health care settings encompassing leadership in patient access/throughput, palliative care, case management, utilization review, and transitions of care programs. Recognizing her ground-breaking work to address health inequities, she was selected for the prestigious Disparities Leadership Program, graduating in 2012 and earning the “best in class” award for KentuckyOne Health. She was invited to serve as faculty for three subsequent DLP classes and to present to the 2015 Institute for Health Improvement annual conference.

Bev is an active member of Kentucky’s statewide task force working to develop a Community Health Worker Coalition. She has volunteered her expertise to support the implementation of a community-based outreach program for seniors living in a low-income neighborhood in Belize, garnering significant improvements for participants in just the first year.

Bev holds certifications in healthcare quality and case management and is a Lean Sigma Green Belt. In addition to her professional work, she is an active social outreach advocate in Louisville, serving on the board of the Family Community Clinic and the Northeast Family YMCA.

Patients spend most of their time outside the doctor’s office – at home, at work or at school. Entrepreneurs, payers and providers are recognizing that healthcare has to move out into the “real world” to be successful. In this session, you’ll hear how a data company is working with firefighters to reduce non-emergency 9-1-1 calls and reduce the burden on the ER. An entrepreneur will explain how home visits and video visits are improving access to care, and a healthcare veteran will explain how the FDA approved physical therapy via motion trackers to get rehab patients off the couch.

Senior Vice President and Chief Executive Officer, Sharp Coronado Hospital

Dr. Susan Stone is the senior vice president and chief executive officer of Sharp Coronado Hospital, a 181-bed, full-service community hospital specializing in total joint replacements and advanced liver care. Sharp Coronado is a nationally recognized center of excellence in patient-centered programs and services. In addition, Dr. Stone oversees Sharp’s clinical laboratory services across the system.

Dr. Stone joined Sharp HealthCare in 1986 and has held numerous leadership positions, including chief nursing officer for two of Sharp’s seven hospitals. Dr. Stone successfully led the 2007 and 2012 Planetree Designation efforts at Sharp Coronado Hospital and Sharp Memorial Hospital, respectively, in addition to the 2013 Magnet Redesignation efforts at Sharp Memorial Hospital.

Active in the Association for California Nurse Leaders, she is currently a Southern California regional representative to the state Board of Directors and serves as a regional Nursing California Action Coalition Champion. Dr. Stone is known for innovative patient-centered care program development, and is a national Patient Centered Care Fellow. She contributed to the development of the Designated Patient Centered Care Hospital Program. Dr. Stone’s patient-centered research has been featured in the peer reviewed Health Environments Research and Design Journal. She was also a contributing author to the 2008 Patient-Centered Care Improvement Guide.

She earned her bachelor’s of nursing science at San Diego State University, a master’s in science and a doctorate of philosophy in nursing science at the University of San Diego.

As a researcher, he has published 1100 peer-reviewed articles, with over 165,000 citations, elected to the National Academy of Medicine, and is one of the top 10 most cited researchers in medicine (Thomson Reuters ISI, “Doctor of the Decade”). His principal scientific focus has been on the genomics and digital tools to individualize medicine. Prior to coming to lead Scripps STSI in 2007, for which he is founder and principal investigator of a flagship $33M NIH grant, he led the Cleveland Clinic to become the #1 center for heart care and was the founder of a new medical school there. He has been voted as the #1 most Influential physician leader in the United States in a national poll conducted by Modern Healthcare. Besides editing several textbooks, he has published 2 bestseller books on the future of medicine: The Creative Destruction of Medicine and The Patient Will See You Now.

Breakout: Using Data to Make End-of-Life Discussions Easier

Talking about death is never easy. Whether it’s a child engaging with a parent about end-of-life wishes, or a clinician approaching a patient about his end-of-life care options, the conversation is difficult to approach. Informed, timely and compassionate conversations are key to ensuring patients receive the right care at the right time. A Nashville analytics company has built an analytics-based technology, Medalogix Bridge, that uses EMR data to identify which patients could most benefit from hospice care. The Bridge system also has a workflow component that helps the care team collaborate and discuss care options with each other, and if appropriate, with patients and family members. In this session, you’ll hear how the system was developed and how one provider is using the solution to improve end-of-life care.

David brings nearly 20 years of healthcare leadership experience to the Mission Healthcare team, beginning with his six years of United State Navy service. As a Navy corpsman, David treated both sailors and Marines in combat zones. The experience instilled a deep passion to serve others in their time of need. Following his military service, David worked in West Africa as a member of an OB/GYN surgery team, further deepening his desire to lead wherever he may be called.

Following his work in Africa, David managed and led medical practices, focusing on a client and patient first customer service model to develop customer and patient loyalty and revenue growth. With this experience, David transitioned into sales and development within the senior living industry, working in development for a nationally recognized provider of senior living, homecare and hospice. David eventually rose to the helm of this organization as the executive director for their private duty care management and home care division in San Diego. Following this experience, David joined Mission Healthcare where he started as field sales professional before taking his current position as Mission’s regional manager of development.

He has worked as a cardiovascular research fellow previously at the Scripps Translational Science Institute while completing a Masters of Clinical and Translational Investigation where he conducted several multicenter genomic and digital health medicine trials alongside Dr Eric Topol.

Dr Barrett was the recipient of the A. Menarini Travelling Research Scholarship, the University College Dublin McArdle prize in surgery and has published broadly on the field of precision medicine. He firmly believes that the collision of emerging technologies will radically alter how we target diseases and promote health.

There is no reason for reviews to turn into flame wars or to breach patient privacy. He is a 2015 Health Policy Fellow the Center of Healthcare Research and Transformation and advises health service researchers and state policy makers on the effects of health care legislation to the business community.

Challenging “standard of care” in 2005, Conor’s treatment protocol was determined by diagnostic markers versus relying solely on clinical pathology sparing him from excessive treatment. Today, Conor remains cancer free with no detectable long-term side effects.

Realizing the unmet need to expedite promising medical discoveries for pediatric cancer, Beth Anne co-founded The Nicholas Conor Institute (TNCI,www.TNCI.org). The Mission of the Institute is to ACCELERATE, DEVELOP, and DEPLOY innovative cancer treatments for children, adolescents, and young adults.

In working closely with multiple sectors (academia, industry, non-profit, government, and patient advocacy groups), Beth Anne identified industry barriers that prevent the transition of promising discoveries for pediatric cancers from academia into a research and development pipeline and ultimately into the clinic. She developed innovative business strategies to address these barriers directly and passionately advocates for the creation of public-private partnerships to support rare disease and childhood cancer translational medical research, changes in regulatory policy, and incentives to encourage industry to invest in small patient population markets.

After treatment for breast cancer, I saw an announcement on Facebook from the Young Survival Coalition, looking for state leaders. I knew nothing of the work they had done; I just winged it as usual. Once I met with other survivors like me, I saw that I had accepted my body post-treatment better than other ladies seemed to have. So I had to help, beyond the YSC. My husband and I started Haus of Volta, a nonprofit collective aimed at promoting self-love and body acceptance among survivors, and others.

After completing the course she not only became a mentor for the hospital, but she volunteered to be a mentor for of the FCSN. Debbie wanted to give back to the FCSN after they were there for her and her family. After years of being a mentor she was asked to be the wellness coordinator in 2012.

Dr. Andrew Renda’s work includes leading strategy, analytics, metrics and research related to measuring progress towards Humana’s Bold Goal of improving community health 20% by 2020.  CDC’s Healthy Days survey instrument is used as the primary means of assessing population health.  This is supported by clinical leading indicators and a robust research agenda that drives insights on how best to improve community health.

Previous roles have included advancing clinical models of care through development, implementation and evaluation of population health initiatives aimed at preventing and delaying progression of chronic disease.  This included product and benefit design, as well as health projects ranging from messaging campaigns and self-care interventions to clinician-led disease management programs. Significant initiatives include: Chronic Condition Special Needs Plans (C-SNP), Metabolic Syndrome Support Service, Asthma and COPD disease management and self-care programs, Sleep Apnea diagnostics and management strategy, Flu and pneumonia campaign, Tobacco cessation service integration and outreach.

He also serves on several Humana clinical and research committees, including: Humana’s Clinical Leadership Network, Clinical Quality Metrics Alignment Governance Committee, Comprehensive Health Insights Joint Research Governance Committees, and Humana Publication Committee.

Dr. Renda has a B.S. in psychology and biology from the University of Kentucky where he was a National Science Foundation Undergraduate Fellow.  He received his medical degree and a diploma in clinical psychiatry from the Royal College of Surgeons in Ireland, followed by a Master’s in Public Health from Harvard University.
